Call for Nominations: National COSH 2013 Health and Safety Activist Awards 

Instructions for Nominators

COSH Awards Categories

Tony Mazzocchi Award: Lifetime Achievement Award: Tony Mazzocchi (1926-2002) was the most influential labor leader in the Occupational Health and Safety field in the U.S. This award gives recognition H&S activists fighting for the H&S rights of workers.

New H&S Activist Award: This award recognizes a recent activist into the COSH network for their sustained and outstanding efforts and dedication to fight for improved health and safety for all workers. 

Educator/Trainer Award: This award recognizes the accomplishments of an individual or group of individuals who excel in reaching workers though training and education and therefore empowering workers to fight for improved health and safety.

Other: If you know someone who is doing great work in Health and Safety, send us your description of their work—awardees need not fit into one of the above categories.

Please provide:

• Your (nominator’s) name and contact information (including email address);

• The nominee’s name, organizational affiliation, and contact information (including email address); 

• Which of the awards you are nominating this individual for (may be in the “other” category);

• Whether you believe he/she would be able to be at the Awards Dinner to receive the award; and

• Why this nominee should receive the award for which you are nominating them. (Please provide a statement of no more than 300 words to be used during the selection process and for the awards program if that person is selected.)
